The Intermediate Care Service is a multi-disciplinary team of health and social care professionals working together to help residents live fuller, more independent lives. We provide fast-paced, responsive support to people in their own homes or residential settings following a hospital stay or crisis in the community. Our service supports individuals for up to six weeks as they recover from illness, surgery or injury, helping them regain confidence and independence on their journey to recovery.
<h3>Key responsibilities…</h3>

<li>Work as part of an integrated, multi-disciplinary team.</li>
<li>Provide and work within a planned programme of rehabilitation, care and support for people who use the service following an assessment of their needs, including personal care, meal preparations, exercises and mobility, cognitive therapy and therapeutic moving and handling.</li>
<li>To monitor and make observations of general behaviour, abilities and responses of people who use the service; record and report information in relation to the person’s situation and review out comes and risk assessments as the person’s condition changes. Maintain accurate records.</li>
<li>Undertake statutory and mandatory training, participate in identifying own training needs, attend training programmes and contribute to the induction of new staff within the service. Develop and maintain own competencies in all areas of care, support and rehabilitation.</li>

Working in a trauma-informed and psychologically informed environment, you’ll empower residents to achieve their goals and improve their well-being. You will ensure the smooth running of the night service by conducting welfare checks, supporting residents, and handling any issues that arise. <h4>About the Role</h4>
We're looking for an experienced Registered Practitioner Psychologist with a background in clinical psychology to join our team in Croydon. The service offers a psychologically informed environment across four sites, providing low to high support forensic mental health services to our residents with forensic mental health needs, complex conditions, and dual diagnosis. In this role, you will drive the delivery of evidence based psychological assessments and interventions aimed at the rehabilitation and reintegration of our residents into the communities. This is a creative, evolving, and impactful role which works closely with our Group Psychological Lead and wider teams to provide support to staff and at times residents to support in providing a streamlined and psychologically informed service.
Key Responsibilities include:&nbsp;

We're looking for someone who is a self-starter, able to proactively encourage, take leadership and ownership for creating a psychologically informed environment. You will be able to communicate highly complex and sensitive information effectively within the organisation and with external partners, collaborating with external stakeholders regularly in a multidisciplinary approach. You will be able to utilise your own expertise and support in empowering the wider team to enhance their skills to deliver a service which is supportive, safe, and in line with best practice for the organisation and external regulatory bodies.&nbsp;

<li>Post-graduate doctoral level training as a Registered Practitioner Psychologist including models of psychopathology, clinical psychometrics, and neuropsychology, two or more distinct psychological therapies and lifespan developmental psychology</li>
<li>Previous experience working as a qualified clinical and/or forensic psychologist, including working with people with complex backgrounds/needs and a good understanding of the sector we run our services in</li>

The Post
We are seeking an&nbsp;Advanced Practitioner Occupational Therapist&nbsp;to join&nbsp;Brent’s Reablement Team—an integrated service that combines fast-paced responses to&nbsp;Home First&nbsp;discharges with the delivery of reablement and social care services through a short-term intervention model. The team’s ethos centres on promoting and enabling independence, and we maintain strong working relationships with our partners and colleagues in the hubs and intermediate care services.
This is an excellent opportunity for an experienced Occupational Therapist with a background in working with adults with physical disabilities and older people. The successful candidate will play a key role in supporting the team by providing specialist advice, guidance, and supervision to non-qualified staff (OT Assistants). In addition, the practitioner will have the opportunity to work directly with Brent residents in their homes, carrying out OT assessments and delivering reablement interventions.

The Person
The ideal candidate will hold a relevant professional qualification, such as a Degree or Diploma from the College of Occupational Therapists (or equivalent), and must be currently registered with the Health and Care Professions Council (HCPC). Evidence of recent and relevant Continuing Professional Development (CPD) is also required.
We are looking for a&nbsp;Advanced Practitioner Occupational Therapist&nbsp;with excellent communication skills and experience working in a social care or community-focused multidisciplinary setting. You should have a strong understanding of moving and handling equipment, and be confident in recommending both minor and major adaptations.
You will be passionate about supporting vulnerable adults and committed to delivering high-quality services. Strong skills in multitasking, prioritisation, caseload management, and time management are essential.
This role offers the opportunity to work as part of a multi-skilled team, including Social Workers, Occupational Therapists, Occupational Therapy Assistants and Care Assessors.
The successful candidates will ideally have post graduate experience in Social Care and/or Intermediate Care, have experience of supervising staff and or students in the work place and of working with Home Care providers and/or Reablement providers.
