You will be working in a team of 5 other social workers and will be always supported by an assistant team manager and team manager, where you will be offered regular formal and informal supervision and support. St Helens practice improvement team also works alongside teams to offer support in developing practice and there will also be lots of training opportunities to develop your practice further and to support achieving your future goals in your career.<br><br>The Duty team is made up of 4 teams in total meaning you will be on duty receiving referrals and progressing assessments for children and their families one in every 4 weeks. You will have experience of undertaking child protection investigations, supporting children in need and developing plans to reduce risk and improve outcomes for children and young people. You will work alongside many partners in developing these plans some of which will be the police, health, and education. You will be able to shine and show your excellent communication skills and be great at being organised, your direct work with children will show and demonstrate a child’s lived experience which will be central to your planning and support to families.<br><br>This is a great opportunity to join the St Helens team. St Helens Council Emergency Duty Team (EDT) also provide AMHP cover (out of hours) and often Mental Health Act Assessments are passed over to EDT from daytime or, from EDT to daytime services which impacts on both services.<br><br>The successful post holder will bridge the gap where day services end and, EDT begin, providing a continuum of day services from 16.00 to 20.00 hours, five days a week. This post will firm up the statutory responsibilities under the Mental Health Act 1983. It is hoped that the post holder will take the work that on occasions is passed from service to service. This will ensure that we provide a smooth-running service to our service users and NHS colleagues in Mental Health Services, as referrals for Mental Health Act Assessments requested later in the afternoon would be coordinated by the successful post holder, rather than waiting for EDT who arrive on shift at 17.00 hours. The Adult social work/Occupational Therapy teams are responsible for the Short- & Long-Term delivery of person centred support and an asset-based care planning/ equipment provision for Adults over the age of 18 years.</p>\n<p>Halton Borough council’s expectations are high, and you will need a strong commitment to providing first-class person-centred care to Halton residents and support to their carers. You will be passionate about working with a range of service user groups -older people, adults with Mental Health difficulties, adults with learning disabilities and adults with physical and sensory difficulties, challenging discrimination whilst promoting independence.</p>\n<p>The Mental Health social work team works with adults, across 2 teams, 18-65 years & Older People Mental Health Teams, who meet the criteria in line with the Care Act and require support in relation to their mental health. We work closely with our Mental Health Trust colleagues to provide a responsive, holistic and recovery focussed response to adults with complex mental health needs.</p>\n<p>The teams work closely with The LLAMS team, Home Improvement Agency, Joint Equipment provision, Independent Living Centre, Continuing Health Care Nurses , CPN’s, Specialist Learning Disability Nursing Service, Neuro-Rehabilitation Team, GP Practices including District Nurse services, Community Physiotherapy and Occupational Therapy.
