You will join a team which consists of a Service Manager, Deputy Service Manager, a day team of 10, and a night team of 4. You will be a present member of the team on the night shift, with a minimum of 2 people working per shift. We are here to promote independence and give our residents the tools needed to fulfil their lives in the community independently.
In this role, you will play a key role in supporting our residents on their journey to recovery, reablement, and rehabilitation, you’ll support individuals transitioning from complex care or acute mental health services to a structured rehabilitation setting. The Integrated Discharge Service assesses Hackney residents, whilst in hospital across all London Hospitals and beyond, for the most appropriate and safe discharge destination and care and support plan.
We focus on safe discharges that respect the legal rights and expressed wishes of all individuals.
The team works in partnership with the multi-disciplinary teams within hospitals and attends multi-disciplinary meetings on their aligned wards.
The service is inclusive to those in acute settings who are assessed as having a learning disability, mental health diagnosis, or dementia. It also supports residents with no recourse to public funds who have eligible care and support needs.
The service operates seven days a week, and staff are invited to contribute to weekend working if they so wish.
Community Reviews are completed collaboratively with the resident, family members, carers, and other involved professionals, to ensure that their long-term care and support needs are assessed and that any newly arising concerns or needs are addressed.
The team is separated into a Homefirst and Reablement team and a Complex casework team, with a Team Manager leading each pathway.
Homefirst and Reablement Team Manager (IDS)&nbsp;- Leading a Team of social workers, Occupational therapists, OT assistants and Review officers to support residents in hospital to receive a timely discharge via either the reablement pathway, a restart of their Package of care or a new care and support plan. Working closely with partner agencies including the NHS, Age Uk, Housing and Extra Care
Complex Casework Team Manager (IDS) -&nbsp;Leading a Team of social workers who are working with residents in hospitals with more complex needs who may require safeguarding, an interim placement or a short term Reablement assessment flat in order to safely meet their needs. The Team Manager will work closely with their counterpart in IDS to ensure a consistent approach in practice and operational processes. In addition, the team manager will work closely with partner agencies reinforcing an MDT approach. and have a sound knowledge of the legal framework relating to Care Act Assessments, Human Right Act, Mental Capacity Act.

The Post
Shared Lives is an alternative to traditional types of care and support available to adults with a learning disability. In Shared Lives, a person lives, or stays with a Shared Lives carer within the local community. The Shared Lives carer provides the care and support in their own family home and as part of their own family and community network. The person would become part of the carer’s extended family and be supported by the Shared Lives carer to live the life they choose.
Hackney Shared Lives supports people who rely on the help and support of others to maximise their potential and maintain their sense of independence.
Hackney shared lives scheme is a CQC (Care Quality Commission) registered service offering the opportunity for vulnerable adults to live in the community. The adult is “matched” with a carer who is recruited, assessed, trained and supported by scheme officers.
The role of the registered manager is to support this process and ensure the compliance of the scheme within the remit of CQC. Each placement is different and this brings challenges and rewards.The registered manager is responsive to CQC, Shared Lives plus (who offer oversight of all 150 schemes nationally) and also works alongside the multidisciplinary Mental Health and Learning Disabilities teams who refer to the service.
Shared Lives Carers are self employed and supported to ensure their fees are competitive and vacancies are advertised. The team has enjoyed a period of stability, including the restructure of the scheme financially and procedurally. The scheme is ready to grow and increase the offer of community living to vulnerable people in Hackney.
The Person
Whilst we value experience, you will need to be committed to providing quality service and opportunities to the people living within Shared Lives. You will need to demonstrate your understanding of supporting people on the Autism Spectrum and Learning Disabilities. You will need to evidence leadership qualities, flexibility, enthusiasm, initiative, and resilience, for what is often a very demanding but rewarding vocation.
Your role will include being a CQC registered manager for the Shared Lives Service, supervising the scheme officers, supporting carers and vulnerable people, organising relevant training, providing reports to various stakeholders and being prepared for the possibility of a CQC inspection. The registered manager takes the lead role for the scheme in matters of safeguarding. The service is always aiming to be “outstanding” with CQC and take an innovative approach to social care and quality assurance improvements - which is a key role of the registered manager. Hackney is a culturally diverse borough and the scheme works to support and recruit potential carers from all parts of the community - building relationships with the community groups is essential. The scheme has a high number of out of borough placements for which the registered manager will need to have oversight and work with regional commissioners.
